Culinary Health Fund

The Plan offers two dental plans: Dental Plan A or Plan B.
You and your family need to use the same plan.
You can change plans every year during dental enrollment, and once in between enrollments.

	Dental Plan A "capitated" or "closed" plan	Dental Plan B "open" plan
Who you can use for care:	Choose a dentist who is contracted by our dental plan. Click here for a current list of Plan A dentists.	Choose any licensed dentist you like.
Plan Limits	No maximum yearly benefit. No deductible.	$1,500 maximum benefit each year.
Are Braces Covered?	Yes – for children. You pay $850 over 2 years.	No
Regular Check ups and x-rays	Usually free to you.	Little cost to you.
Costs for other care	Most services have very little out of pocket costs to you. See Dental Plan A Copays.	Costs you more. The Fund pays a smaller amount, and you pay the rest. Click here for a list of what the Plan pays.
