Your employer reports your work hours to the Culinary Workers Health Fund.
Before you can receive health benefits for you and your family, you must meet initial eligibility rules.
Your employer must make contributions to the Fund for your work hours.
You must enroll your dependents for coverage.
To enroll your dependents:
Spouse - bring a certified copy of your marriage certificate
Children - bring a certified copy of each birth certificate, or proof of legal guardianship and any other legal documents showing who is responsible for medical care, such as a Qualified Medical Child Support Order.
Domestic Partner - (of the same sex), you must enroll for benefits together in our Customer Service office.
