As a new employee:

  • Your employer reports your work hours to the Culinary Workers Health Fund.
  • Before you can receive health benefits for you and your family, you must meet initial eligibility rules.
  • Your employer must make contributions to the Fund for your work hours.
  • You must enroll your dependents for coverage.

To enroll your dependents:

    • Spouse - bring a certified copy of your marriage certificate
    • Children - bring a certified copy of each birth certificate, or proof of legal guardianship and any other legal documents showing who is responsible for medical care, such as a Qualified Medical Child Support Order.
    • Domestic Partner - (of the same sex), you must enroll for benefits together in our Customer Service office.
