I didn't work enough hours in a 2-month work period to keep my Culinary benefits. How do I make a self-payment?
You can self-pay in person at Customer Service or by mail. You can pay by cash, a check or money order or credit card (Visa and MasterCard). There is also a "drop box" in Customer Service if you come after the Office is closed or on weekends.
How do I know how much self-payment I owe?
You should receive a notice in the mail telling you how much you owe. If you do not get a notice, but think you are short hours, you should call Customer Service to check on your hours. It is very important that you tell the Fund whenever your address changes so that you receive your self-pay notice and other important plan information.
When are self-payments due?
Self-payments are due by the day before each eligibility period starts. For example, a self-payment for May/June coverage is due by April 30th (see chart below).
