Your
Spouse’s Eligibility
This is your last chance.
If you are married and have not returned your “Spouse Health
Insurance Verification Form”, your dependent spouse’s eligibility
that was terminated on January 1, 2007 will remain terminated and
their claims will be denied. Your spouse’s eligibility will
remain terminated until you complete and submit this form to
us before January 31, 2007.
This form must be completed for all
dependent spouses.
If your spouse does work for a non-contributing
employer and is eligible for health benefits at their work, they
have until January 31 to enroll for benefits at their job to remain
covered as a dependent of the Culinary Health Fund. Your spouse
should take the “Certification of Group Health Plan Coverage” form we sent to you to their employer.
Under the HIPAA Portability Rights, your spouse has the right to a
special enrollment through their employer to accept late enrollees
if you request enrollment within 30 days.

office before January 31.

The Culinary Pharmacy at its new
location!
The Culinary
Pharmacy moved to a new building just a few doors away.
- There are no changes to your prescription benefit.

All medications at this pharmacy will cost you nothing.

Your script will transfer to the new location.

The business hours are the same - Monday to Friday from 8am to
6pm and Saturday from 10am to 2pm.
Come and visit us in our new location! Remember, all drugs in

To log on to the Catalyst Rx website, enter the eligible
employee’s Social Security number as the “Member ID” and the date of
the birth in the “Members Login” box located on the right side of
the screen. Then click “Login”.


Does your spouse work?
Effective January 1, 2007,
if your dependent spouse is eligible for health benefits from a
non-contributing employer (with at least 2,500 employees in Las
Vegas), they must enroll for benefits at their job to remain covered
as a dependent of the Culinary Health Fund.
Your children do not have to be covered under the other insurance to
remain Culinary dependents, only your spouse.
To
verify if your spouse is affected by this eligibility rule change or
for the list of the non-contributing employers, please contact our
Customer Service office at (702) 733-9938.
Forms needed to ensure your spouse remains a dependent are being
mailed to each household the week of December 4th.
You
must return the form (Spouse Health Insurance Verification Form) we
sent to your home for your spouse to remain eligible. If you did
not get this form in the mail,

Otherwise, your spouse’s Culinary coverage will be terminated on
January 1, 2007 and claims will be denied.
This form must be

identified, your spouse may need to take this form to their employer
in order to enroll for health benefits at work. Once enrolled at

LabCorp for your
laboratory services
Just a reminder that LabCorp is the only lab provider you should
use for your lab tests. If you use other labs such as Quest for lab
work, you will have to pay for your services.
LabCorp just
opened another location.

Get your FREE flu shot!
It’s that time of the year! Get a FREE flu
shot from Culinary. Free flu shot available for all eligible
Culinary participants and dependents.

You and your eligible dependents 14
years and older, can now get your flu shot from participating
Albertson’s (SavOn) Pharmacies by appointment or walk in. Click

Children under 14 years of age can
get their flu shots from our Flu Clinic on October 28. The
Clark County Health District nurses will provide flu shots to children
under 14 years. If you bring your children’s shot records, you
can get any needed immunizations at the same time on
October 28th by the Clark County Health District
nurses.

Eligibility Rule Change -
Effective January 1, 2007
Eligibility rule change coming in January
2007. If your spouse works for a non-contributing employer with at
least 2,500 employees in the Las Vegas area, please make sure you
read the following:

Your spouse will need to take advantage of their
employer-offered health insurance in order to remain an eligible
dependent on your Culinary Health Plan.

Make sure your spouse enroll for single coverage. Your
dependent children are not be affected by this eligibility rule
change.

You will need to provide enrollment proof to the Culinary Health
Fund.

Culinary Care Assistance Program
Update
Starting
July 1, 2006, Harmony is
the new benefit manager for the Culinary Care Assistance Program.
There are NO changes to your benefits – you will get the same
great benefits now provided.
What is
Changing?
-
A new phone number to call when you
need mental health care. The new number is on the back of your
new prescription card -
(702) 251-8000.
-
You will now be
able to make calls for a family member or other loved one in
need of care.
-
You will have a
‘rapid response’ provider who will ensure that Culinary patients
are seen quickly during a crisis.
-
Many of the
same providers are in the Harmony Network. You also have the
option of using the two Harmony mental health clinics if you
prefer and they have a large provider network from which you can
choose.
-

health issues, self-assessment tools and wellness information.

New Self-Pay Rate, effective June
1, 2006
The new contribution rate for Health benefits is increasing 3
cents per hour this year. The rate increases from $3.28 to $3.31 per hour for hours starting in June, 2006. The self-pay notices
mailed to you
in August are for May/June work months, and will include the old rate of $3.28
for May hours, and the new rate of $3.31 for June hours you are short. The self-pay rate is reviewed each year, and matches the rate paid by your employer.

NEW Culinary ID and prescription
news
The Culinary Health Fund is pleased to announce
that Catalyst Rx will manage your prescription benefit program
effective June 1, 2006. Other than your new medical/prescription
card, there are no changes to your existing prescription
benefits.
Watch for your new Culinary
medical/prescription cards in the mail. If you do not get your
cards in the mail by the end of May, we probably do not have your
correct address. To get your new cards, please call our
Customer Service office at (702) 733-9938. With your new card,
let your covered dependents know that they will need the covered
Employee's Social Security Number to give to the pharmacy or doctor.
Social Security Numbers have been taken off the cards to protect
your privacy.

new pharmacies to the Culinary network. And you now have
access to more than 55,000 pharmacies nationwide.

REMINDER! LabCorp is
the ONLY contracted Culinary Provider for laboratory services.
Culinary
patients using other labs such as Quest for labwork will have to pay

now has eleven lab service centers in Las Vegas. LabCorp and
the Culinary Health Fund are working together with your doctors to
get your lab work orders moved to a LabCorp service location.

GENERIC
FORM OF OXYCONTIN
The generic form of Oxycontin is now available
in all doses. As you know, generics are safe, effective, and FDA
approved. They just cost less than brand name drugs.
To control the rising costs of prescriptions,
your Plan will only cover the generic, once a generic is available.
Your doctor may still prescribe Oxycontin if he is unaware of the
new generic. If so, the pharmacist will change your brand name
Oxycontin prescription to the generic version. If you do not allow
a generic substitution for Oxycontin, you will have to pay the
difference in cost between the generic version and the brand name.
If you have medical requirements preventing your from taking the
generic form, you and your doctor will need to submit medical
evidence to the Fund justifying the use of the brand name versus the
generic drug.

NEW 24-HOUR
URGENT CARE CLINIC
As of December 1, 2005, the Southwest Medical Associates
Urgent Care Clinic on Rancho Drive is part of the Culinary provider
network. This is a 24-hour Urgent Care Clinic. Your
copay will be $35 only at this urgent care location.

Please remember
that other Southwest Medical locations are NOT part of the Culinary
Network, and will be paid at out-of-network rates, which will cost

Please also
remember that while Urgent Care clinics provide convenient services
after hours, and can help you avoid an unneeded emergency room
visit, they are not designed to replace your regular doctor. And
Urgent Care visits cost you more than a regular doctor visit.
