Culinary Laboratory Services Providers

As a reminder, LabCorp and Quest Diagnostics are the Culinary Health Fund’s contracted laboratory services providers.  Your Culinary patient now have more options to get their laboratory services.  There are 17 LabCorp locations with 1 open 7 days a week and 17 Quest locations with 1 office open 24 hours.

You will need to contact LabCorp for any specialized lab tests that are not offered by either LabCorp or Quest.  LabCorp is assisting the Fund in monitoring and tracking any laboratory tests provided by an out-of-network laboratory provider.  To order a test not performed by either LabCorp or Quest, you must contact LabCorp at 602-454-8053 prior to ordering the test.

For a list of lab tests available by these Culinary contracted laboratory services providers, visit the LabCorp or Quest website.

Prescribe Generic Medicines

As the Fund works hard to control rising prescription costs and protect our participants health benefits, we urge you to help your Culinary patients by:

  • prescribing generic medicines and/or
  • prescribing medicines available at our Free Pharmacy if appropriate.

The Fund opened a special pharmacy in Winter 2003 only for Culinary Health Fund participants. Certain generic and brand drugs can be filled in the Free Pharmacy at no cost (FREE!). This is a great way to reduce prescription costs. Click here for the “Free Drug List”.

If you prescribe generic medicines to your Culinary patients, they would only have to pay the $5 copay.
