Effective June 30, 2005, NovaCare Outpatient Rehab is closing its physical therapy practice.  Your office should have received notification to assist you with referral needs.  Click here for an updated list of our PPO Therapy Providers.  You may refer to any of these therapists for patient care at PPO rates and Culinary Health Plan copayments.
