The Culinary Health Fund is dedicated to providing quality health care to its participants. Part of that dedication is giving physicians and their staff the tools they need to make the administrative part of the health care experience as seamless and efficient as possible for the physician, participant and Culinary Health Fund.

As a result, the Fund has developed the Provider Manual to assist physicians and their office staff to navigate the Culinary Health Plan benefits on behalf of its participants with simplicity and ease.

In the manual you will find helpful information regarding claims submission and payment, prior authorization, pharmacy and much more.

Information in the manual will be updated regularly. Updates will also be provided right here on the Fund’s website.
