What is the Diabetes Reward Program?
It’s a program that offers classes for you to learn how to live better with diabetes.
How do I get my reward?
Attend our classes and get the check-ups listed below:
  • Diabetes Class #1
  • Diabetes Class #2
  • Kidney Smart Class
  • Foot check: call your podiatrist (foot doctor)
  • Eye check: call your ophthalmologist or retina specialist (eye doctor)
For each class you attend, you get a stamp on the passport book and a reward. You also get a reward after you complete the program. It celebrates you attending all the classes and getting all the check-ups.
Where are the classes held?
The classes are held at 515 East St. Louis Avenue, Building C, Las Vegas — inside the Culinary Health Fund Education Room.
How do I sign up for the Diabetes Reward Program?
Call the Advocacy Line at 702-691-5665 to sign up for the program.
Where can I get medicine and supplies for my diabetes?
  • The Culinary Pharmacy can provide you with diabetes medicine and supplies at no cost.
  • View the Culinary Pharmacy Booklet for a list of medicines in your plan.
