If you don’t work the 240 hours that are required in the two month period, you can pay the Culinary Health Fund for the hours that you are short.


If you only worked 220 hours during the two month period, you are short by 20 hours. You can pay the Culinary Health Fund for the 20 hours you are short to keep your benefits.

How will I know if I am short on hours?

You will get a Self-Pay notice in the mail. You must make your self-payment before the due date to keep your coverage. (See chart below)

How do I Self-Pay?

  • Call our Automated Phone System at 702-733-9938, then select #3 to pay by phone.
  • Send a check or money order in the mail to:

  • Drop it in the Customer Service Office drop box.
  • You can also come by the Customer Service Office from 7:30 am to 6 pm (Monday through Friday) to pay with cash, credit card, check or money order.

Things to know about Self-Pay

  • You cannot Self-Pay to get Initial Eligibility.
  • For every hour you are short, you pay the same amount that your employer pays.
  • Self-Payments are due at the end of every even month: February, April, June, August, October, December.
  • You may only make 6 full self-pays in a row (1 year).
  • If you don’t do Self-Pay you will lose your eligibility. To become eligible again you have to start the process all over again: work 360 hours during a 3 month period.
