Patient Portal instructions

Get your test results from the online Patient Portal

If you don’t already have an account, be sure to register for the Patient Portal to access your test results online. You may need to send the results to your employer before going back to work.

You can also register by following these 6 easy steps:

1. Visit our Patient Portal at https://culinaryhc.com/covidresults/
2. Click “Sign up” to create a new account.
   
3. Enter your name, date of birth, gender, email, and phone number.
   
4. Choose if you want to get your temporary passcode by email, call, or text.
   
5. Enter the temporary passcode.
   
6. Create your new password. It must have at least:
   • 8-20 characters
   • 1 uppercase letter
   • 1 lowercase letter and
   • 1 number
