Eligibility

I just became eligible for Culinary Health Fund benefits. How do I make sure my family is also covered?

My employer did not report all of my work hours.
What do I do?

You can call the Customer Service Office at 702-733-9938 to check on the work hours reported for you by your employer. If not all of your hours were reported, notify the Customer Service Office and they will contact your employer to verify the hours.

I went on vacation, but my vacation hours were not reported for the work period I was gone. What do I do?

Employers usually pay you before you go on vacation. If they count those hours when they paid you, instead of when you were gone, you have too many hours in one work period and not enough in the next. You can contact the Customer Service Office at 702-733-9938 and explain when your vacation hours were taken. They will contact your employer to verify when your vacation was to determine if the hours were credited to the right work period. If not, your employer will move them to the right work period. It is important that you contact the Customer Service Office right away so your coverage
is not disrupted.

What is a self-payment?

It’s an option for you to keep your coverage when you have worked less than 240 hours in the two month period. The Culinary Health Fund let’s you pay for the hours that you are short.

How do I know how much my self-payment is?

You should receive a notice in the mail telling you how much you owe. If you do not get a notice but think you are short hours, you should call the Customer Service Office at 702-733-9938 or check your hours on our website by clicking here. It is very important that you tell the Culinary Health Fund whenever your address changes so that you receive your self-pay notice and other important plan information. It is your responsibility to update your
contact information.
