Where can my employees go for chiropractic care?
Your employees can go to any in-network PPO chiropractor. We only pay for services from an in-network PPO chiropractor.
What will my employees pay for chiropractic care?
|$15 copay, then we pay 100% of the remaining allowable charges.
|$30 copay per visit, then we pay 100% of the remaining allowable charges.
We only cover some chiropractic services. Your employees will have to pay all of the costs for any non-covered services they receive.
