Copay information

Your employees are responsible for paying copays when they visit the doctor or get a medical service. Copay amounts depend on the type of service and where they get care.

We work hard to give your employees access to the best healthcare and to keep it affordable. That’s why there are many services for your employees that require no copay.

How do my employees check their copays?

It’s important for your employees to be aware of their copays so they know what to expect when they get care. The Culinary Health Fund Co-payment BookDownload PDF can let them know how much they’ll pay for different services.

Copays can change. If your employees need help understanding their copays, they can call our Customer Service Office at 702-733-9938.

Dual Culinary

If your employee and their spouse both have Culinary coverage, then they only have copays at the pharmacy.

Co-insurance

Some services have co-insurance. Co-insurance is the percentage of the cost of a covered health care service that an employee must pay.

Example

  • The allowed amount for a medical service is $100.
  • If the co-insurance for the medical service is 20%, your employee pays $20.
  • The plan pays the rest of the allowed amount.

You can see which services require co-insurance in the Co-payment BookDownload PDF.
