In-network dental care

Your patients must get their dental services from a Culinary network dentist in the Nevada Dental Benefits (NDB) group for this plan.

Your patients can find an in-network dentist by visiting NDB’s website and choosing “NDB Managed Care Plans” in the Find A Dentist section.

Not all dentists who say “Culinary is accepted here” are in our network! Your patients can call NDB at 702-478-2014 to check if a dentist is in the network. This can help them avoid extra costs.

Orthodontics (braces) Covered for your patients and their eligible dependents. Look at the table below to see how much your patients will pay.
Dual Culinary coverage If a patient and their spouse both have Culinary dental benefits, they and their eligible dependents will not have any copays.
General anesthesia Covered for some extractions
Missed appointments Your patients should call their dentist the day before their appointment if they have to cancel.
The plan will not pay any fees for missed appointments.

Are orthodontics (braces) covered by the plan?

Orthodontic coverage is provided depending on the diagnosis. Treatment for cosmetic purposes is not a covered benefit.

Your patients’ copays for braces from a Culinary network dentist:

Age Copay
For children (under 19 years old) The total copay is $850.

  • $300 is due when the braces are put on
  • Then, it’s $50 per month for 11 months

There may be additional costs if:

  • Treatment will be longer than 24 months or
  • Your patients need other appliances such as a device that moves or positions the teeth during treatment
For adults (age 19 and older) The total copay is $3,100.

  • $900 is due when the braces are put on
  • Then, it’s $50 per month for 36 months
  • Plus a copay of up to $400 for a retainer at the end of active treatment.
